Complete Atlantis prescription form.

Make sure to include ALL of the following information on Rx:

• Return date
• Shipping/ Billing info
• Implant Data
• Phone/ Fax #
• Answers to ‘KEY QUESTIONS’
• Special instructions
• Doctors/ patient names
• Check boxes for additional services, provisional shade, special screws etc.

Step 2

Package:

Please package & ship the following items with your implant case to Atlantis:

• Atlantis Prescription Form
• Study Cast or Vinyl Polysiloxane (VPS) Impression
• Opposing Cast or VPS Impression
• Bite registration
• Index of Implant Fixture

Please note that all items listed above must be included to avoid delays in processing your case.
