How to Order - 3 simple steps

Step 1: Complete Atlantis prescription form.

Make sure to include ALL of the following information on Rx:

• Patient name or reference number

• Prescribing clinician's zip code

• Astra Tech Customer number (if available)

• Shipping & Billing information

• Implant data

• Material selection

• Margin data & design

• Healing abutment data (if present)

• Design option selection

• Additional case instructions

Step 2: Package

Please include the case material detailed below to avoid delays in processing your order.

All Cases:

• Articulated Master Casts with analogs placed
• Bite Registration (recommended)
• Model of Soft Tissue (silicone or stone)
• Completed Atlantis Prescription Form

Cases with 4 or More Adjacent Edentulous Spaces:

• All the above plus:
• Removable Diagnostic Preparation that relates to Master Cast (Atlantis will scan this diagnostic and use it to design the abutments.)

Full Arch Cases:

• All the above plus:
• Occlusal Bite Rim or Record Base (recommended)

Securely package the case material in either the CaseSafe™ box provided by Atlantis™ or your own shipping box. If using your own box, please ensure that each model or impression is securely packaged with at least one layer of bubble wrap to avoid damage during shipment. Secure the box flaps with strong packing tape.
